Live-Scan Fingerprinting Procedures

  1. Background Check/Fingerprinting Request Form:  https://adeaels.arkansas.gov/AelsWeb/Consent/Consent1.aspx The form MUST be completed online, printed off, and brought to Fingerprinting appointment.
  2. Transaction ID #: https://www.ark.org/ade/licensure_cbc/index.php.You must bring the transaction ID # receipt to your appointment.  
  3. Child Maltreatment Form:  https://ardhs.formstack.com/forms/dcfs_central_registry_request_v2  The form MUST be completed online before finger prints will be accepted.
  4. Print all forms and bring your government issued photo ID- You must bring a government issued ID to your appointment.

Finger-Printing is available by appointment only.

Livescan Fingerprinting is a service for School Districts in our area provided by the Northwest Arkansas Education Service Cooperative. IF you are not employed by one of our school districts, you will be charged a $10 fee. Cash, check and credit cards accepted.
